Tongue-tie or ankyloglossia is a developmental anomaly of the tongue characterized by an abnormally short and thick lingual frenulum, resulting in limited movement of the tongue. Ankyloglossia can affect eating, speech and oral hygiene, in addition to having mechanical and social effects. The diagnosis of tongue-tie is based on a clinical examination. The mobility and appearance of the tongue associated with the insertion should be evaluated, as well as the insertion and shortening of the lingual frenulum. The management of ankyloglossia should be considered at any age considering the risk-benefit assessment and due to the greater vascularity and mobility of the tongue; lingual frenectomy should be performed with fewer traumatic events to avoid postoperative complications. The objective of the present study was to describe, through a clinical case report, the technique of conventional surgical lingual frenectomy performed in a pediatric patient who had an anomaly in the lingual frenulum and impaired his phonation. It is concluded that the frenectomy surgery was able to return part of the normal functions of the tongue to the patient and its benefits will be increased with the continuity of the speech therapy treatment.
