THE ROLE OF CONE BEAM COMPUTED TOMOGRAPHY IN THE DIAGNOSIS OF CORONARY FRACTURES WITH PULP EXPOSURE: CASE REPORT

Introduction: This case report describes the management of traumatic apical periodontitis resulting from a motorcycle accident. Dental trauma is a common condition, and accurate diagnosis, especially with the aid of advanced technologies such as CBCT, is crucial for successful treatment and functional rehabilitation. Objectives: To present the clinical approach and therapeutic success in the treatment of traumatic apical periodontitis in teeth with pulp exposure, highlighting the importance of cone beam computed tomography (CBCT) in diagnosis and planning. Case Report: An 18-year-old male patient presented with crown fractures and pulp exposure in teeth 11 and 12 resulting from a motorcycle accident. Negative pulp sensitivity tests, along with positive palpation and percussion, and thickening of the apical periodontal space visible on radiographs and CBCT, confirmed the diagnosis of traumatic apical periodontitis. Treatment included chemical-mechanical preparation and intracanal medicament with calcium hydroxide, followed by root canal obturation. Discussion: CBCT was essential for assessing the extent and location of periradicular lesions, enabling precise treatment planning. The initial success of endodontic treatment, evidenced by the absence of symptoms and adequate obturation, reinforces the effectiveness of the approach employed. Long-term follow-up is essential to monitor periapical healing. Conclusion: This case demonstrates that traumatic apical periodontitis can be effectively treated with appropriate endodontic intervention, aided by advanced imaging such as CBCT. The absence of symptoms and the patient's functional improvement indicate a favorable prognosis.
