Dental self-extraction associated with the absence of dental care may contribute to the progressive worsening of oral conditions and functional impairment. This case report aimed to describe the clinical treatment of a patient with severe oral health impairment associated with multiple residual roots, history of dental self-extraction, and social vulnerability factors. A 39-year-old male patient attended the dental clinic presenting persistent pain, masticatory difficulty, extensive tooth loss, and multiple infectious foci. Initially, oral environment adequacy procedures were performed, including prophylaxis and periodontal scaling. Subsequently, treatment was conducted through serial tooth extractions performed in different clinical sessions. During follow-up, adequate tissue healing, absence of infectious signs, and improvement of oral conditions after removal of infectious foci were observed. The case highlights the importance of oral environment adequacy and surgical intervention in the dental management of patients with severe oral impairment and associated vulnerability factors, contributing to the improvement of oral health conditions.
