Extraction of a paramolar supernumerary tooth in the maxilla: a clinical case report

Supernumerary teeth represent a developmental anomaly capable of altering occlusal harmony and periodontal health, requiring accurate diagnosis and appropriate treatment. The objective of this study was to report the extraction of a supernumerary tooth classified as a paramolar, emphasizing the selection of surgical maneuvers based on tridimensional tomographic analysis, with focus on anatomical criteria, root proximity, and the risk to the integrity of tooth 26. This clinical case refers to a 27-year-old female patient who presented to the Dentistry Clinic of IAES–Amazonas on August 13, 2025, reporting the presence of extra teeth for extraction. Clinical examination and periapical radiography revealed a supernumerary tooth located in intimate contact with the distal region of tooth 26, which exhibited only a radicular remnant. The supernumerary tooth showed morphology compatible with a premolar, thus being classified as a paramolar. Considering its potential to cause occlusal and periodontal repercussions, surgical extraction was defined as the therapeutic approach. The results confirmed the importance of imaging diagnosis and preoperative planning for successful extraction, reinforcing that integrated clinical conduct and technical precision are essential for functional and aesthetic outcomes. This report contributes to the clinical and scientific understanding of the management of paramolar supernumerary teeth, highlighting the relevance of individualized treatment planning and continuous professional development.
