Full-mouth disinfection protocol in a patient with periodontal disease: a case report

Periodontitis is a multifactorial inflammatory disease that affects the supporting dental tissues, resulting in progressive loss of clinical attachment and alveolar bone resorption when not adequately treated. The aim of this study was to report the management of a 62-year-old male patient diagnosed with stage III, grade B, generalized periodontitis, using an intensive protocol of Complete Mouth Disinfection (CMD) combined with intensive local antimicrobial therapy in two weekly sessions. The treatment was associated with local antimicrobial therapy using 4% metronidazole gel and 0.12% chlorhexidine gluconate antiseptic. The treatment consisted of scaling and root planing with Gracey curettes and an ultrasonic cavitator. Repeated application of metronidazole gel was standardized to 10 minutes of action, followed by antisepsis with 0.12% chlorhexidine in three consecutive cycles per session. The protocol was finalized with the application of neutral fluoride gel for enamel protection. After treatment, a significant reduction in bleeding on probing, improvement in the gingival inflammatory pattern, and reduction in tooth mobility were observed. These findings indicated a favorable clinical response to the intensive protocol employed. In conclusion, FMD associated with local antimicrobial therapy and fluoride is a viable and effective therapeutic tool for the management of patients with generalized chronic periodontitis, offering speed, controlled intensity, and favorable short-term clinical results, provided it is complemented by rigorous patient adherence to the maintenance program and continuous oral hygiene.
